The family and/or caregiver continue to provide care same as before but now with the assistance, support and direction of the Hospice team.
"Home" is considered to be wherever the patient lives. It can be a personal residence, board and care, assisted living home, or nursing home. If needed, the Hospice team can help with facility placement or respite stays.
Hospice services are covered by Medicare, Medicaid and many insurance companies.
1) The patient, family or caregiver can call for a free consultation with a Hospice representative. 2) Referrals for Hospice care can also be made by:• Physicians• Hospitals• Nursing Homes• Assisted Living Facilities 3) The patient and/or family can then decide to elect Hospice services.
